Alabama can decertify you for the staffing you cannot hire. Turnover runs past 100 percent.

When turnover exceeds 100 percent, you are not staffing a program. You are running a hiring operation that happens to deliver care.

The number

The interim evaluation of Alabama Community Waiver Program, prepared by Health Management Associates for the Alabama Department of Mental Health and revised May 7, 2026, measured direct support professional turnover at 118 percent in personal and habilitation services and 105 percent in residential services.

Above 100 percent means the average position turns over more than once a year. Every training requirement, every competency check, every personnel file is being rebuilt continuously, and the organization never reaches steady state.

What the state does with that

The ADMH Comprehensive Provider Manual for CCBHCs lists among the changes requiring immediate notification: "Inability to provide one of the required CCBHC services due to workforce shortages or other capacity limitations. Inability to accept new individuals for CCBHC services due to workforce shortages or other capacity limitations."

You must self-report when the shortage stops you delivering. And the same manual lists among decertification and termination grounds: "Failure to maintain the appropriate staffing, required licensures and certifications, and availability and accessibility of services."

Read together, those two provisions describe the trap. The state acknowledges workforce shortage as a real condition, requires you to disclose when it bites, and retains staffing failure as a ground for decertification.

The training gate before anyone works alone

Alabama Administrative Code Rule 580-2-20-.03, effective September 14, 2025, requires that "All employees/staff having direct service contact with recipients shall be trained in the following prior to working alone with recipients: Management of aggressive/assaultive behavior. Crisis intervention techniques."

Prior to working alone. At 118 percent turnover, that is a gate your scheduling system hits every few weeks, and the failure mode is a well-meaning supervisor covering a shift with someone who is not cleared yet.

The scale underneath it

ADMH reports roughly 350 certified providers delivering about 1,700 services, serving 232,000 individuals in fiscal year 2025. Federal HRSA data shows 67 mental health shortage area designations in Alabama with about 28 percent of need met.

This is a small provider network carrying a large population in a thin labor market. Employer brand is not a marketing luxury here. It is the constraint.

Four things to change this month

1. Treat recruiting as a standing operation, not a response to vacancies. Above 100 percent turnover, the pipeline has to run continuously or you are always behind it.

2. Make the works-alone clearance a hard scheduling flag. Not a training record someone checks. A flag the schedule will not let you override.

3. Write the notification procedure before you need it. If a shortage stops a required service, you owe ADMH immediate notice. Decide now who sends it.

4. Put the decertification language in front of your board. They should read the staffing ground in the state own words, once.
